Effect of unilateral pulmonary artery obstruction on circulatory dynamics in dogs with chronic atrial septal defects.

چکیده
IT IS generally accepted that the left-toright shunt across an atrial septal defect is the result of a pressure gradient in this direction between the two atrial chambers during most of the atrial cycle. The maintenance of this pressure gradient and, hence, the direction of the shunt in the left-to-right direction has been attributed to a difference in the volume-elastic properties of the two atria and to a difference in the distensibility of the two ventricles." In addition, previous investigators have demonstrated by means of indicator-dilution techniques, that both in patients' and in dogs with atrial septal defects located in the region of the fossa ovalis, a higher percentage of blood from the right lung shunts in the left-to-right direction across the defect than from the left lung. This preferential shunting of blood from the right lung has been interpreted as being due primarily to the anatomical proximity of the openings of the right pulmonary veins into the left atrium and the position of the atrial septal defect in the atrial septum. Fleming found, on the basis of bronehospirometry of separate lungs with the subject in the supine position, that an abnormally high proportion of oxygen is taken up from the right lung in patients with atrial septal defects of the secunduni type. These results suggest that a greater than normal portion of pulmonary blood flow traverses the right lung

متن کاملEffect of atrial septal defects on hemodynamic alterations caused by acute partial obstruction of pulmonary artery.
P investigations of the hemodynamic effects of progressive constriction of the pulmonary artery have demonstrated the basic principles of right ventricular compensation for an increase in resistance to outflow." Acute obstruction of the pulmonary artery results initially in an increase in right ventricular diastolic volume and in increased vigor of cardiac contraction, with maintenance of cardi...
متن کاملThe relationship between pulmonary artery wedge pressure and left atrial pressure in man.
In 17 patients with atrial septal defect, in 12 patients with mitral stenosis and in four patients with unilateral lung tumor but without evidence of mitral valve disease, a similarity in pressure pulse contours in the left atrium and pulmonary artery wedge position was obtained at cardiac catheterization. It is concluded that the pulmonary artery wedge pressure pulse is a reasonably accurate r...
متن کاملChanging hemodynamics in patients with transposition of the great arteries.
Follow-up cardiac catheterizations were carried out on 65 patients with transposition of the great arteries. Patent ductus arteriosus (PDA) was a significant problem in this group only when closure resulted in a precipitous drop in arterial oxygen saturation. Spontaneous closure of ventricular septal defects (VSD) occurred in approximately 20% of patients while natural or surgically created atr...
متن کاملHolt-Oram Syndrome: A Rare Variant
Holt-Oram syndrome is an autosomal dominant disorder, characterised by skeletal abnormalities of the upper limb associated with congenital heart defect, mainly atrial and ventricular septal defects. Skeletal defects exclusively affect the upper limbs in the preaxial radial ray distribution and are bilateral and asymmetrical. They range from clinodactyly, absent or digitalised thumb, hypoplastic...
